So, I was testing this mainly using cloned humans (/mob/living/carbon/human/clone), but I just took a look at the code. Looks like a mob's name variable is used for designating enemies, so I guess designating is based on appearance name (ex. Unknown, by wearing a gas mask, rather than the person's true name).

Looking at the wiki, it mentions being able to change your name to disguise yourself from being targeted as an enemy of the flock.
Though if real_name is checked, this doesn't allow for distinguishing individual mobs for things like critters, and stuff like melting your face with acid changes your real_name to Unknown, meaning real_name checking has problems still.
Not sure if what wiki described was wanted or not, but might be too much of a problem going for that?
