I've attempted to debug the issue but can't trace it back.

There is an issue in disconnecting that I've seen in other music bots that I attempted to fix in this one.

If the bot crashes/dies for any reason on the host, it won't disconnect from Discord immediately (it will eventually time-out though). If the bot is restarted in that window before it is kicked from Discord, then it is stuck in a state where the bot doesn't know that it's already in a channel.

The work-around is that you can change channels, summon it there and then change back and re-summon it to the original channel and of course it will now realise it's in a channel.
