Kobotoolbox is upgraded to docker

Display data collected from the kobotoolbox. Below are the listed API calls to fetch the data for the selected survey,

1. Get the survey question list for “Airoli_Naka_RHS_Navi_Mumbai” survey
   GET https://kc.shelter-associates.org/shelter/forms/Airoli_Naka_RHS_Navi_Mumbai/form.json
2. Gets the collected data for “Airoli_Naka_RHS_Navi_Mumbai” survey
   GET https://kc.shelter-associates.org/shelter/forms/Airoli_Naka_RHS_Navi_Mumbai/api

Default selection can be achieved by

1. Create and populate an external(.csv) file with the name of the city and slum being currently surveyed. (Typically we do lots of surveys once we get to a specific slum, so this does not change once the survey process has started.)
2. Using the pulldata function to populate the default values of the city/slum from the csv file uploaded

To create the csv file, build a simple html5/js web page to populate and construct the file described in step 1.
