proxy /_api requests to hoodie using grunt-connect-proxy

update hoodie-server to the latest version, it now creates a stack.json file in the data folder, looking like this:

We can read out host and port from www and use it to proxy all requests to /_api on the grunt server to the hoodie server, using https://npmjs.org/package/grunt-connect-proxy.

This is a follow up to hoodiehq/hoodie-server#106
