windows 10/OSGeo4W and elevation grid

On windows 10 with OSGeo4W cs2cs fails to load elevation grid:

This problem was reported by István Kecskeméti.

Using only the horizontal grid no error (etrs2eov_notowgs.gsb).
On Linux (Ubuntu 16.04) no error using Rel. 4.9.1. or Rel. 4.9.2
On windows QGIS 2.18 can read/load both grids (horizontal and vertical), I suppose files are not corrupted.
The bug may be in the cs2cs 4.9.3 or only in the Windows versions. I did not tested 4.9.3 on Linux and 4.9.2 on Windows.

Submitting grids to proj-datumgrid

@zsiki Would you be interested to submit the grid(s) here to the https://github.com/osgeo/proj-datumgrid repository so they can be used more easily by PROJ and its downstream consumers?

The procedure to contribute is documented at https://github.com/OSGeo/proj-datumgrid/blob/master/CONTRIBUTING.md. Otherwise imitating a pull request like OSGeo/proj-datumgrid#89 + OSGeo/PROJ#1791 is probably the easiest road to follow.

I see there's a https://github.com/OSGeoLabBp/eov2etrs/blob/master/LICENSE with GPL v2. It is fine for software, but for datasets, I'd rather suggest https://creativecommons.org/licenses/by/4.0/ which is quite popular for geodesic datasets released by other countries.

It would also be great if you could submit the grid(s) for registration to EPSG ( procedure at http://www.epsg.org/EPSGDataset/Makechangerequest.aspx ) , so the transformation info can be directly imported into PROJ database

point code

Point code in the input file should be managed. e.g. in this line
"201;46.69832944;17.15721056;127.16;BUILDING" the sequence of data is as follows: point id; latitude; longitude; elevation; point code
