Make Address an inline of Demographics in the admin?

Might work more nicely with Address as an admin inline of Demographics?

Problems with multiple ProblemDiagnosises

The IDCR template we are implementing uses the same archetype (openEHR-EHR-EVALUATION.problem_diagnosis.v1) to fill two different 'sections': Diagnoses and Problems and Issues.

This seems to become a problem when using the same model twice in Django because it can't work out how to uniquely identify the two instances, or perhaps only one instance is actually being created and bound to the form.

The error message below is raised on running python manage.py makemigrations:

django_openehr_demo.TransferOfCareSummary.diagnoses: (fields.E304) Reverse accessor for 'TransferOfCareSummary.diagnoses' clashes with reverse accessor for 'TransferOfCareSummary.problems_issues'.
	HINT: Add or change a related_name argument to the definition for 'TransferOfCareSummary.diagnoses' or 'TransferOfCareSummary.problems_issues'.

For now I've commented out the second appearance of the ProblemDiagnosis model in class TransferOfCareSummary.
