@dantevvp i was not able to reproduce this issue:

What is the name of your field field in the custom form?
Param name should be id because that's what you use in 'initial data' api path:

@omohokcoj I added a Match field with an id param name. The Teams field still looks empty on all Match records when editing.

This is the response of the API as seen in the Dev console when editing Match 14:

One thing I did notice though is that if I change my Teams field to a select field with a custom query instead of a reference field, it works. But it has to be a select field with a custom query.

Is this expected behavior? Shouldn't the reference field also work for populating initial data without the need of creating a custom query?

@dantevvp thanks it helped to identify the issue - i release a new 0.4.8 version with a fix - hope you can give it a try.
