misc-docs's Issues

Biohazard issues from EHS

Good morning! Just FYI, I posted a consolidated NFPA fire diamond door card for Eng-V 5030 (shared space) this morning.

As a part of hazardous waste management training for yourself and members in your lab group (i.e., when they start), please make sure to have an SOP documented in your lab safety manual binder for Hazardous Chemical Waste ‘Handling, Storage & Disposal’ in your lab(s).

The SOP content can be as follows on a simple word document (copy & paste the content below);

SOP Topic - Proper Hazardous Chemical Waste ‘Handling, Storage & Disposal’ in Dr. Aaron Meyer lab in Eng-V 5031 & 5030 (shared).

“Protocol for disposing of chemical/hazardous waste on campus at UCLA -
All waste chemicals must be disposed of as ‘hazardous waste’ through EH&S. *No chemical can go down the drain. Drain disposal of chemicals is strictly prohibited at UCLA.
EH&S Hazardous Waste Program - https://ehs.ucla.edu/waste/services
*Please login to https://ehs.ucop.edu/waste/#/ and print out the online waste tag.

NOTE: Unlabeled/Unknown containers/Containers with dual or conflicting labels become 'Unknown hazardous waste' and need to be disposed of with a recharge of $65 per unknown container. Recharge form can be found on; https://ucla.box.com/ehs-recharge-order-request
*Please e-mail [email protected] / (310) 206-6612 or (310) 825-0932 or (310) 206-1887 for specific details.

1 All chemical waste containers must be capped/closed when waste is not being added into them.
2. These waste containers must be decontaminated and cleaned on the outside, prior to disposal.
3. To prevent chemical spills/drippings on the outside of the waste receptacle, please use a funnel to transfer/pour contents by placing the receptacle inside a secondary containment.
4. If the existing waste collection container is not in a good condition, the chemical waste in this container must be carefully transferred to an appropriate container that is in good condition without any leaks (i.e., by using appropriate PPE, inside a fume hood, using a secondary container).
5. *Affix WASTe on-line hazardous waste tag on the waste containers right from the first drop of generation/waste collection in the receptacle. *The same is applicable for ‘hazardous sharps’ waste container. *Note: Please mention full chemical name in the contents section of the waste tag. DO NOT mention abbreviation/formulae in this section.
6. Dispose of the chemical waste containers as hazardous waste within 90 days or when full, whichever is sooner. *The same is applicable for hazardous sharps waste container.
7. Liquid hazardous waste must always be stored in a secondary container (plastic tray/tub).
8. Always use an authentic ‘Vented’ cap for piranha solution waste container/any waste container holding content that can build-up pressure.
9. Always use a stable plastic/Nalgene transportation cart with built-in secondary containment for bringing down hazardous waste for routine/weekly EH&S pick-up. *Never transport hazardous waste and/or chemicals without a proper transportation cart.

  1. Do not recap the needles after use. Recapping could lead to serious needle stick injuries. Discard them directly into the hazardous sharps waste container (rigid/puncture proof, non-red receptacle). Do not fill the sharps container more than 2/3rd full at any given time. *Use ‘non-red container’ for non-biohazard sharps/hazardous sharps/sharps disposal. *Affix an online hazardous waste tag on the non-biohazard sharps container and dispose of when about 2/3rd full or within 90 days, whichever is sooner. Do not recap the needles after use. Recapping could lead to serious needle stick injuries. Discard them directly into the designated RIGID sharps container. Post a caution note near the sharps disposal container stating, "Do not recap the needles after use. Discard them directly into the sharps containers"

  2. Dry hazardous waste must be collected in ‘double transparent bags’ and affixed with an online waste tag right from the first point of generation.

  3. Empty containers that had held extremely hazardous substance/waste in them before need to be disposed of as hazardous waste, with an online waste tag affixed on them. *Example: Empty containers that had held; HF, Sodium Azide, Osmium tetroxide, Cyanide compounds, Mercury compounds etc. before.

  4. When the hazardous waste containers are full (i.e., even though not having reached the 90 day disposal duration), they need to be dropped off during EH&S weekly hazardous waste pick-up on Thursdays at 1:45 pm at the Eng-IV building loading dock. *EH&S Hazardous Waste Pick-up Schedule can be viewed at; https://ucla.app.box.com/v/ehs-hazwaste-pick-up-schedule *Arrive 10 minutes before the pick-up time. Do not wear PPE while transporting the chemical waste on the cart in hallways and elevators. Just carry with you a lab coat, gloves and safety goggles.“

NOTE: Please log the training on proper ‘Hazardous waste storage, handling and disposal’ in your lab by using the log sheet found in the link https://ucla.app.box.com/ehs-site-training-record with the training topic being, ‘Hazardous waste storage, handling and disposal’. Please make sure that all concerned personnel and the lab PI will read the SOP and sign this sheet. Please document it in the lab safety manual binder.

Thank you very much for your time and attention in this matter.

Issues from EHS

1). Please login to https://worksafe.ucla.edu and register for ‘PPE Program Fitting and Distribution’ by inputting LAB-PPEDST into the ‘Course Catalog’ search engine there. Please ask your lab personnel to do the same before they start working in the lab. NOTE: During this session, you and your lab group members will be fitted and given lab coats and safety glasses/goggles. Your lab group will be given one face shield (for the group to share) I believe.

2). Please procure 500 grams of Citric acid and 500 grams of Sodium bicarbonate for your chemical spill-kit.

3). Please purchase adequate boxes of disposable Nitrile gloves (i.e., if not done already).

4). Please have easy access to the latest CHP inside the lab (either as a hard copy or online access); https://ucla.app.box.com/v/ehs-chemical-hygiene-plan *Please read the 2017 Chemical Hygiene Plan (CHP) and sign-off on worksafe. Please ask your lab group members to do the same before they start working in the lab.
Directions for verification of the latest Chemical Hygiene Plan through Worksafe
· Go to: https://worksafe.ucla.edu
· Log into Worksafe using your Bruin Online account (BOL) login.
· From the column on the left side of the screen, select Course Catalog.
· Expand the Section titled: EHS-Online Training, by clicking on the folder icon.
· Select the ‘Chemical Hygiene Plan’ module. If your browser uses pop-up blockers please watch for warnings when launching this module. You may need to allow pop-ups from worksafe to complete this verification.
· After you have reviewed the contents of the CHP in the Box window, close the Box window.
· Record your training as finished by signing-off on the window that opens after closing the Box window.
· Please make sure to check your dashboard on worksafe at the end to see if latest CHP sign-off data shows up there.

5). Please add NaOH and HCl on your CIS inventory. Remove CO2 tank from the inventory for room 5031. Please take the abandoned Nitrogen compressed gas cylinder in your lab to shipping & receiving on a cylinder dolly and hand it over to Lennox (Mac).

IMPORTANT NOTE INVENTORY: Check for chemicals in every single lab drawer, wall mounted cabinets/shelving, cabinets beneath the fume hood, inside the fume hood, inside the desiccator/dry box (if any), bench top, lab work bench shelves and all refrigerators/freezers 'in every lab room' and include them on your chemical inventory along with compressed gas cylinders, separately listed & uploaded for each room. *Please make constant updates to the CIS inventory whenever a new chemical/compressed gas cylinder is brought into the lab or removed from the lab. *Please make sure to have accurate '# of items / Quantity & container size' on your CIS inventory for each room. Please double check for chemical name typos on the existing CIS. On your inventory, please list the exact chemical name as it is on the container (i.e., as labeled by the manufacturer). *CAS # for ‘chemical/gas mixtures’ need to be mentioned as MIXTURE.

6). SOP:- Please document detailed protocol for ‘Imidazole’, listing; quantity range, temperature range, why it is used, with what other chemical it is used and how it is used. Please document an SOP with comprehensive lab specific protocol for NaOH and HCl.

7). Please fill out the site safety orientation checklist form and sign it. General note: Site Safety Orientation Checklist needs to be filled out by every lab user (one form per person – including the undergraduates working in the research lab). PLEASE NOTE: This checklist can be found at https://ucla.app.box.com/v/ehs-site-safety-checklist
*This is just required once. Do not leave any item on the checklist blank. If it is not applicable, please mark your 'initials' on the left side and mark 'N/A' at the end of the specific checklist item. *The lab PI/Supervisor needs to sign all the forms.

8). Please print and document IIPP document in the lab.

9). I will be posting the consolidated NFPA fire diamond door card for shared lab space Eng-V 5030 shortly.

10). Please make sure to list Eng-V 5030 as well on your LHAT.
