The easiest way to improve performance is to cache generated ranges. It will make sense in the tasks that generate a lot of regular expressions. For example in transpilers.

Another pain source is a negative ranges. It should be rewritten completely. I even had to write my own wrapper just to avoid this:

regenerate().addRange(0x010000, 0x10FFFF).remove( ranges )

the _subtractRangesFromSurrogatePairsRange method is doing the same thing that you are mentioned in first comment. For example:

plugin._subtractRangesFromSurrogatePairsRange([[0x020000, 0x030000], [0x03FF00, 0x040000]]);
>> [ [0x010000, 0x020000], [0x030000, 0x03FF00], [0x040000, 0x10FFFF] ]
