HI, I've installed the Solution file into my environment and I'm able to access the Entities and Forms. How do I install the PCF and configure it to initiate and complete a survey response?

If I create a Survey with Answers without using an Answer Option Template as follows:

I get the following error:

As soon as I use a template the error goes away and the Survey works correctly.

I have a Model Driven App form that has a lookup field (activesurvey) to choose the Survey Request and the PCF is assigned to the same field. If the User selects a different Survey Request and saves the form they have to then Refresh/Reload the form for the Survey PCF to pick up the change. I've tried adding an onChange handler to the lookup field but I can't find a function on the PCF control to force a refresh.
