Support for Mac OS X
sigtimedwait() does not seem to be available on mac, is there any alternative?
Floppy disk support?

SDL display needs a final update
When using the SDL display and the loop detector, the emulator can drop to the debugger without a final update to the display.
Access to video manipulation from Forth
Defvars for curvid, curline (maybe a defconst for vidram too) would be nice to have in order to implement page (clear screen) and at-xy (output at a particular place in the terminal).
In particular "page" seems a good candidate for implementation via the forth assembler.
Pop from empty data stack returns 46
I haven't started reading your implementation, but this seemed like a bug, compared to other forths I've used.
multiple monitor support
32x12 is pretty cramped. It would be nice to have 4 or 12 monitors tiled together to form a larger screen.
