Could you try apxs -i mod_markdown.la?

UPDATE:
Adding a link to my Xcode toolchains ( sudo ln -s /Applications/Xcode.app/Contents/Developer/Toolchains/XcodeDefault.xctoolchain /Applications/Xcode.app/Contents/Developer/Toolchains/OSX10.11.xctoolchain) and running apxs -c mod_markdown.c did allow me to properly compile.
A couple of caveats about using this on MacOSX 10.11.6. The /usr/libexec/apache2 directory is sandboxed and inaccessible even using sudo, so using the install command on apxs (apxs -ci mod_markdown.c) does not succeed.
I was able to manually copy the mod_markdown.so file into a new directory (eg../usr/local/apache/modules) and run chmod 755 /path/mod_markdown.so.
Then I manually added the LoadModule command to the httpd.conf file in /etc/apache2 using the full path to my compiled module.
Followed this up by resetting apache. Now I'm off to test. Hope this helps the next Mac OS X user.
