Right now, this 500s. User is legit, 0 repos. It's likely crashing on trying to pull the name from the array at posotion 0 (there is no array if no repos).
Rather than adding a row to a DB, the form should integrate with SalesForce to capture the lead appropriately.
This has to be addressed to avoid any issues with people hammering the button
