module.exports causes reference error when calling init on cookieManager module

Bug

Following the steps in the README to import the cookie manager module causes the following error in the console:

cookie-manager.js:439 Uncaught ReferenceError: module is not defined
    at cookie-manager.js:439
cookies:614 Uncaught TypeError: Cannot read property 'init' of undefined
    at cookies:614

To fix, the cookie moanager should be declared globally and the exports module api must be loaded before attempting to export a module. See https://2ality.com/2011/11/module-gap.html for further information about the error.
