Two modes of operation: owner and client

At the moment catflap does things like initialise the index and can be configured to point to a given index. This works well if you are the owner of the acat index.

If you just want to consume the index, this requires you to do some custom configuration, and will issue index init requests against an index that you don't necessarily own.

We should therefore have two modes of operation: the current one, and one where you either can pass in the base-url of the ACAT at runtime (so there's no need to mess with the default settings), or hard-code it somewhere. It should also not automatically run init() - it should be up to the library user to do that.
