imgshow is designed to work in iTerm2. It doesn't work in Terminal.app.

From this answer on Apple.SE, it looks like we'll be able to detect iTerm using the TERM_PROGRAM environment variable, and indeed:

iTerm

% echo $TERM_PROGRAM
iTerm.app

Terminal.app

% echo $TERM_PROGRAM
Apple_Terminal

We should keep two edge cases in mind:

1. TERM_PROGRAM isn't set
2. The user knows their terminal supports this image protocol, but they are not using iTerm2. We should provide a flag like --force-image-support.
