Is it possible to add a "form clearing or reload page function" after submitting?

First thank you for this contact-form !!

When I upload on server your unmodified folder (just changed my email of course) all works great.
My issue is when I include your html form content in my index.html (I added also all the script links) and upload it with js and php files, the sending works well but the confirmation message does not appear in the same page, it opens a blank page with the confirmation message.
Is there any conflict or something I did wrong ? I don't know anything about javascript and ajax.

I just copied your index.php file to my index file and not changed a single line (yet) and I'm getting this error? I haven't changed any of the file/code.

Notice: Undefined index: submit in /public_html//contactus/index.php on line 2

Thanks for this form, I followed your walkthrough and am really pleased with how it's looking.

One minor issue. I'm not massively proficient in .php, and when I test the page the script doesn't seem to run. When I hit the 'Send' button it redirects to a page showing the script. I'm really confused!

Can you suggest where I am going wrong? I've attached some screenshots.
