I'm running iOS 8 build 12A365, all I've done is since installing the app is sync my account.
In the screen shot below, I can trigger multiple white squares on the black background by tapping within the black background.

Can you try to kill the "Scan" app and also rebooting your phone please.

The issue still exists unfortunately. When I first noticed the issue it was shortly after installing iOS 8, and the only app that used the camera was the native app.

Just tested on an iPhone 5 (different model number) and it works there as well. Are you using the version from the App Store or did you build yourself? Is your phone jailbroken?

App Store version and not jailbroken. I don't have any provisioning at the moment for custom builds.
