You have two instances of freak_fortress_2.smx, one in plugins/ and one in plugins/freaks/

Good catch. Unfortunately, when I deleted the one in plugins/freaks and restarted my server, I still got the same error.

Solved it, apparently the issue was that my plugins were set to .smx instead of .ff2.
