A non-operational mailing list GUI using JavaFX.

• Include the following TextFields:
  • First Name, MI, Last Name
  • Address Lines 1 & 2
  • City & Zip
  • Phone
  • Primary & Secondary email.
• Include the follwoing Button nodes:
  • Enter
  • Clear
  • Exit
• Include the following CheckBox nodes:
  • Home
  • Work
  • Cell
• Use a ComboBox for the State.
• Top banner should use Text shapes.
• Include an image.
• Include text effects and colors.
• GUI should be appropriately titled.
