Daily fluid(bed side板板)=奶量+點滴

  • 小於37wks: 80(1st day)→90(2nd day) → 100(3rd day)…150 (max)ml/kg/day
  • 大於37wks: 70(1st day)→80(2nd day) → 90(3rd day)…150 (max)ml/kg/day

Phototherapy時需增加10%的daily fluid

  • 如果要分兩段加的話，大概白班加一次；小夜加一次；也可以直接寫成這樣
  • if 15ml Q3H, then IV keep 4.7ml/hr
  • if 20ml Q3H, then IV keep 3.0ml/hr

U/O: ___ml/kg/hr, normal 2-4 ml/kg/hr

Well baby直接吃全量，

  • 加奶時大部分每天增加5ml，吃的量比daily fluid少時，其餘量由點滴補充
  • (daily fluid-feeding)/24= ? ml/hr 注意單位不一樣

點滴選擇

  • 第一天only給糖 (常用D10W)
  • 第二天：D5-1/3S or Tai 5 (確定尿有出來)
  • 觀念：NBR會給得較密集 NBR Q3H餵；BR Q4H餵

當病人:baby:IV keep line時

  • 每個藥物4ml/dose
  • e.g. ampicillin Q8H+gentamicin QD:point_right: 4x4= 16 ml/day

當病人:baby:IV for drug時

  • Ampicillin 5ml/dose, gentamicin 25ml/dose, vancomycin 4ml/dose, claforan 4ml/dose
  • e.g.1: ampicillin Q8H+gentamicin QD:point_right:15+25= 40ml/day
  • e.g.2: vancomycin Q8H+claforan Q8H:point_right:4x6=24ml/day
