ok, it's a kind of race condition, when a block mining starts and transactions are preprocessed accountantTree.updateAccountNonce is called twice - incrementing and decrementing account nonce.

In turn TransactionsPendingQueue._removeOldTransactions is driven by the 20 seconds timer. If its call appears in between two actions above - transaction validation fails.

I added extra logs to see how nonces are validated

no transactions were mined in between but for some reason accountantTree.getAccountNonce() returned another value and validation failed.

I re-created transaction from the same address and account nonce was 25 again.
