The GPIO pins on the raspberry PI are 3.3V and you shouldn't draw current from them (max 3mA), this is not enough for powering any standard 5V fan (even very small ones).
The common way is to have a FET between the fan and the GPIO, however I am not using RPI4 for GPIOs, I suggest to look at community forums, there are even ready made kits for that.

The fan works perfectly fine using an always on 3V3 pin though? Or does that not matter when it comes to GPIO

The GPIO can't supply the current for the fan.
