Since applying these changes I'm seeing spam (mail with X-DSPAM-Result: Spam headers) show up in my inbox. @lukecyca, how about you?

@al3x I haven't had that problem. X-DSPAM-Result: Spam is going directly to the Spam subfolder for me.
Could you capture a relevant segment of /var/log/mail.log and post it or email it to me?

This ended up being due to a mail client swapping out my Spam folder for a Junk folder. Our .dovecot.sieve only moves spam into a folder named Spam, and will fail open to delivering mail to the Inbox if it can't find a folder with that name.
