I also have this issue.
App version : 4.0.4
OS : Windows 7 Pro SP1
Edit: I have 2 screens and only the first screen has this small square in the upper left corner, the second screen has a full screen square that does job (only on this screen though).

I have the same issue.
App Ver.: 4.0.4
OS.: Windows 7 64bit Professional
Monitors.: 3 > only on the middle monitors does not work?

Yes, im working with 4 Monitors.
3 are working properly, the main one has only the tiny square in the upper left corner
from desktop-dimmer.
