Can you paste output of "heat event-list -n 2 stack-name"? The fact that docker volume is being deleted during stack creation suggests that something weird is happening (maybe nova instance creation is failing?). "Expecting to find username or userId in passwordCredentials" might be caused by stack operation running too long so keystone token maybe expired (only guess), or it might be a heat issue.
