This fix may have triggered a new bug.

This is my full .htaccess file:

RewriteEngine On

force the www subdomain

RewriteCond %{HTTP_HOST} ^[^.]+.[^.]{2,4}$
RewriteRule .* http://www.%0/$0 [R=301,L]

This is a catch-all rewrite rule that seems to work on all URLs ... except on "/". I
expected "example.com/" to redirect to "www.example.com/", but it doesn't. On the
other hand, "example.com/foo" issues a 301 to "www.example.com/foo" just fine.
