I've been having the same issue ever since upgrading to Opera 12. I've tried reinstalling Opera as well as the add-in. You click the icon and it opens up the pop-up which just shows a blank gray square, then Opera crashes. I'm running Opera 12.00 on Windows 7.

This appears to have been resolved as of Opera 12.12 on x64 linux. Yes!! ..I can finally start using the extension again.
