The only time I’ve ever experienced this is when the authentication to devops expires. I usually just open the image in a new tab, and the authentication kicks in and everything starts working again. For some reason devops uses a different domain for the images which causes a separate authentication to happen

Thanks @kyledodge. The profile pictures are loading again but I didn't do anything to make it work. Next time it happens I'll try your workaround.
I wonder if anything could be done in the extension to resolve this. Maybe I'll go dig if/when I ever get a bit of time.
