No actions on save configured. Occasionally it's reliably reproducible even on restart. If that happens I will attempt the troubleshoot bot.

I wonder if this issue is being caused by an installed extension. Can you try to run VS Code without extensions? From the command line (NOT the integrated terminal in Code), execute: code --disable-extensions and try your steps again to see if it reproduces. If you see it is an issue with the extension, please file it against the extension repository itself.

I haven't yet been able to repro without extensions.

When I launch without extensions, it works. But then when I launch back into my IDE with extensions, it also works.

It is at some random time later my IDE begins to not be able to save new files. I'll continue trying every now and then.

For my use case, I'm pretty sure I narrowed it down to this extension:

I manually bisected, and I went back and forth with this extension ~5 times and successfully reproed 5 times in a row.
