If you open the command palette, can you search for live preview commands?

Are you on an html file?

@andreamah I installed it a few minutes ago and I have the same issue. I'm on html files, I tried several different ones.

There is no button where it's shown in the readme

It works through the command palette thank you! I don't think often enough about trying the palette, I'll remember that

Same for me. No Live Preview context menu, or live preview icon.
The command palette shows the option to start the Live Preview server (and it opens the preview panel) but it doesn't refresh on page changes, and even if I click the refresh button inside the preview it doesn't load the updated page. The option is activated:

It's a local environment.

Strange. If auto-updating is not working, this means that the websocket server isn't starting. What do you see in the console when you run Developer: Toggle Developer Tools?

I came in here with this same problem hoping for a solution. I see lot of other complaints about this with no answers. This is the error I get in case it is the same thing the others are seeing. This occurs when I click on Show Preview. I unfortunately don't have time to troubleshoot this at the moment.

Does this get fixed when you try an earlier version?
