Now I see the mistake.

Now working:

The reason was the cue file:
correct: TRACK 01 MODE2/2352
wrong: TRACK 01 MODE2/2336

Weird, loads fine here (same version of PCSX2 core):

Sure you converted it properly? I used the redump version of the game (bin/cue format) and chdman from MAME 0.228 with the command line chdman createcd -i "Gradius V (Europe) (En,Fr,De,Es,It).cue" -o "Gradius V (Europe) (En,Fr,De,Es,It).chd"

Yes, sure. I've used the newest chdman 0.231 with the same command line.
I used a working iso. This iso and a converted gz file both work, but not the CHD file.
RA is crashing/closing, no boot logo or anything else.
CHDs of other games work.

What bios do you use ? With or without fast-boot ?

I have scph70000.bin (JP BIOS), scph70004.bin (EU) and scph70012.bin (US) in my "bios" folder, my core option was set to use the JP BIOS and it worked fine with Fast Boot. With Fast Boot OFF however it doesn't seem to work with the JP BIOS selected, but it boots fine with the EU and the US ones.

I ended up with Gradius V (Europe) (En,Fr,De,Es,It).chd, MD5: 820727F585F14EBFB1061BCD11E5253B

I'd say chdman doesn't like your original file? No idea why tho, but that's the only thing I can think of :p
My original files are Gradius V (Europe) (En,Fr,De,Es,It).bin and .cue, MD5 of the .bin: 02A60F25A9C2CACF986C4AE7AA137208

Tried standalone PCSX2 now:

Must PS2 DVD and CD images always be this ?
TRACK 01 MODE2/2352
Or are PS2 DVD images different ?
I want to be sure what is correct and what isn't.

No problem, glad you got it to work!

Idk much about the CD modes and all, the only other game in CD format I have is "Maximo - Ghosts to Glory", and it's also MODE2/2352, so I guess it's needed for these games. The DVD games I have were .iso before converting, without any .cue file.

Maximo - Ghosts to Glory is a DVD, how you can see at the cover.
And Gradius V is a CD.
You can make a bin+cue or iso out of every image, DVD or CD doesn't matter.

I'm not really sure about the cue files.
Shox is a CD like Gradius, but has this cue: TRACK 01 MODE1/2048 But it works fine.
I just changed it for testing to TRACK 01 MODE2/2352 like with Gradius and it doesn't work anymore.
So ... ???

But if a CHD isn't working, I know what to check now 😁

Oh, maybe the EU version then, because of the multiple languages included, etc. but the US version I have is a CD :D

Not sure about the mode, maybe it depends on how it was originally dumped, but the redump cues (http://redump.org/cues/ps2/) are all MODE2/2352 at least.

Oh, you're right.
Seems I've seen a wrong selfmade cover having DVD on the front.
US is CD, EU is DVD, correct.

That's so strange.
All my CHDs are MODE1. But they all work fine.
I'm very lucky because I thought I have to convert them all.
Why does it care for Gradius V and Shox working only as MODE2/2352?
