I would suggest to change the xpath to use the "for" attribute for the zip(s) and company_name labels.
It seems to work with this modification.

I found another same kind of issue with:
<xpath expr="//div[@class='col-md-12' and ./h3[contains(.,'Shipping Address')]]" position="attributes"> as it is language dependant.
