A first step might be aria-live=assertive, and some sort of encapsulation so the text doesn't get reread as it counts down.

So generally, role="alert" works great; however as you already pointed out, when you have things like countdowns inside the alert content, it will great read over and over, and not much seems to care about aria-relevant for that.

Setting role="alert" on just message-inner means the action buttons wouldn't be read (which I think would be pretty important/relevant).

The easiest compromise I would think of would be to move the phrases into their own div, after the message and the action buttons, so the alert role can be applied to both message and buttons without including the phrases.

That said, I don't actually use a screen-reader day to day, so I'll ask some people who do and then follow up here.
