Right now it'd be difficult because, as you mention, binding to anything other than the platform-assigned port results in pain. The reason is because the IP address is shared across all other dynos on the same runtime host. There's some work in progress to fix that by handing out per-dyno IPs, which you'll be able to use as pseduo-loopback interfaces to your heart's content. We don't currently have an ETA, unfortunately.

The new version of PHP support on Heroku uses Apache 2.4 or Nginx with PHP-FPM (or HHVM, if you like!). The FastCGI interface is over unix sockets (supported in Apache's mod_proxy_fcgi since 2.4.8).
