What exactly does not work? In particular:

  • what steps to execute to reproduce it?
  • what results do you observe?

Have a file with the words - Developers and developers in the same order in 2 consecutive lines. Place the cursor on the first line and press * for forward search.

what results do you observe?

The cursors stays at Developers. The count is [1/1].

what results would you expect?

It should move to the next line to developers as I have set ic in my .vimrc. The count should be [2/2].

Ah, so this is related to the * command... This is happening because the plugin reimplements * and # to work around some Vim issues. Please see the docs:
https://github.com/google/vim-searchindex/blob/master/doc/searchindex.txt#L31

You can configure case-sensitivity for * (independently from the other search commands) through plugin options:
let g:searchindex_star_case=0
