I agree that the current way that we add another, say observation, shouldn't be done with a pop-up. Dialog boxes are not generally used in that way. It would be much better if was an activity which contained a listview of his previous observations, with a floating action button allowing us to add another observation. That way, the user can view his previous observations and then move on after making sure he has included everything. The do not add button does not even have to be there.
Even though there is a button on the actionbar which summarises your form details, I think it would be more visually coherent for the user if the above were done.
