I have just tried to put heterogeneities everywhere in the outer-core with dvp inside the cylinder and 0 outside. The model would look like this (where red dots are for dvp=10% and black dots are for dvp=0):
In this case, the output VTK files for model_vpv_*.vtk is:
It looks like I need to fill the entire outer-core to create the heterogeneity that I want. That can be a solution...

It might actually be true, that the heterogeneity region is always interpreted as an interval in colatitude and depth, which would explain your last finding and the fact that the het_gll region in your original post was a half circle. I see, that this makes more sense in the mantle than in the core, but if that works for you by adding a few zeros, I would not do anything about it...
