I took a quick look at the codepen, and I saw none of the gform code, no reference to a Google Spreadsheet ID, and the javascript from this repository wasn't listed anywhere in the dependencies. In order to use this form correctly, you need to create a google spreadsheet and publish a google script for it with your email, see the tutorial in the readme. Thus, the email gets sent by Google's API, which you use javascript to submit the form data to Google's API. If you have further issues to discuss here though, just let me know. I am closing the issue for the moment since I could not find anything connected to the code in this repository.
