I confirmed that Sale type was being used. I found the answer to this issue here: http://stackoverflow.com/a/9983587/202168
Basically my sandbox seller account had USD as primary currency and my shop was sending payments in GBP... and by default the sandbox account requires payments in foreign currencies to be manually confirmed.
The solution is to log into PayPal sandbox as the seller, go to Profile > My Selling Tools and change the following option from 'Ask Me' to 'Yes':
