Well, the three dashes you see is because you're viewing the site on something that's smaller than a desktop (i.e. tablet/phone). If you click on the three dashes, you should find the menu items.

Wow, that's crazy... yeah, surely enough expanding my browser window size a
bit did the trick. Still on 'tablet sized' logout then login doesn't work
properly: try it... (tested on MacOS X, firefox 22, using 1/4th of my
screen).

As for the login, can you check the console to see if it's giving you any errors? How is it failing?

Sorry if that wasn't clear in the book, but that menu just uses Bootstrap's responsive menu code (which is in turn generated by the bootstrap-accounts package). We wanted to really focus on Meteor-specific stuff, not on front-end coding and responsive design, so we didn't dwell on this too much.
That being said I just tested on Firefox on Mac OS and was able to log in / log out fine even with a small window. Could you take a screenshot of what happens when it's not working?

Sorry for the delay, I just took a look. I haven't been able to reproduce the issue, but it looks like a problem with bootstrap's responsive CSS code.
Changing the app's CSS code is a little bit cumbersome because it forces us to re-tag each commit every time. So since this is not a major bug and it's not really related to Meteor, I'm afraid we will have to leave it like this, at least for now. Sorry about that :(
