I used the https://github.com/starzel/contentexport example to add widgets to my own form and make other tweaks for my specific needs:
Add a copy of the 'export_content.pt' template to the templates folder, and add
template = ViewPageTemplateFile("templates/export_content.pt")
to the example CustomContentExport, and enjoy.
