Hmm, are you using an Eyefinity setup or something? I use 3 monitors but the main monitor is always starting at 0x0 so the tool works just fine. I would assume that a setup where Windows treats both Monitors as one big monitor could lead to problems if you do not stretch to the full size of both monitors ?!
But maybe setting the position manually to 0 and 0 could help?

My unofficial fork has a patch that lets you specify a position now. You may want to try it and see if it resolves this issue with Eyefinity.

I have my main screen on the right side, so i expect that it will not start at 0x0.
Often games get maximised on the wrong screen.
It would be best if it were possible to choose which device to use, i think.

All of these issues are resolved in recent builds that let you select monitors, draw regions, and specify exact coordinates as desired.
