I've merged a bunch of stuff from @zixaphir's v3 branch including the fix for this.
Although the most recent version from @zixaphir seems to update the captcha when and only when the selected post is removed, which I don't understand the reasoning for.

And the reason it does it only when the selected one was removed is actually in error, though if I were to say there were theory behind it, it would be because if you're currently typing out a post, you wouldn't need the captcha at that particular moment, and a fresh captcha has the least possibility of expiring.

Unfortunately not everyone was specific with their complaints, but #29 lists some issues. Although I haven't yet tested them myself, so I can't be sure what's my fault and what's broken in your v3 branch.

There were also some issues which I fixed in 1.6.1, namely the header disappearing (#24) and Quoted Title not working at all.

Zixaphir's fix for this was retained in 1.7.0, so this should be working. Presumably the issue from 3 days ago was just #44?
