do you get any related errors in the error-log?
did you run the upgrade-process to create the migx-config-table: http://rtfm.modx.com/display/ADDON/MIGX

does it work when you set the modx-system-setting 'debug' to 0 ?

the debug is "0" and i created the actions as listed in rtfm but before i could click on upgrade button it loads the homepage with errors. here is the error log details, seems config table is not created

the migx-config-table isn't created before you run the setup-script from MIGX-Management as explained on rtfm.
But you can't do so when the MIGX-Management doesn't run.

Did you choose migx-index from dropdown for the action when creating the menu?

tried removing migx package and installing it again but no luck! the only warning upgrading to modx 2.2.2 was about suhosin.
