I've just pushed 0.3.16 - but, I'm not holding my breath it will fix this issue.

I'll have to figure out how to replicate your setup.

It stems from overriding the views, inside checkga/show:

<%= f.text_field :gauth_token, :autocomplete => :off%>

I've noticed the migration generator has the following:

change_table :users do |t|
  t.string  :gauth_secret, :gauth_token

is this correct? It does not seem to do anything with the gauth_token.

Changing these to gauth_secret worked but then resulted in an 'invalid barcode' error inside the Google Authenticator app.
